J.C. Blair patients are now able to access their medical records through a secure internet portal. This valuable tool will enable patients toconveniently view, manage, and download certain medical records such as lab test results, procedures, prescribed medications and hospital bills.
J.C. Blair, along with all hospitals across the United States, is required to make more meaningful use out of available technology that allows patients to more easily access their medical records. Having this electronic access will allow you as the patient the ability to review your medical history after your stay at J.C. Blair.
The J.C. Blair Memorial Hospital Patient Portal relates to services provided at J.C. Blair only and will not include health information from any other health care facilities patients may utilize for health services.

What is the patient portal?
A patient portal is a secure online website that gives you convenient 24-hour access to your personal health information and medical records—called an Electronic Medical Record or EMR—from anywhere with an Internet connection.
How do I sign up?
When a patient is registered or admitted at J.C. Blair, the registration staff will ask for the patient’s email address and provide information on the patient portal and instructions on how to enroll. To sign up, the patient must go to the hospital’s website www.jcblair.org, click on the Patient Portal link on the left side of the home page, and enter their email address and medical record number. Patients must have a valid email address on file at J.C. Blair and their medical record number to enroll.
How can I access my medical record number?
The medical record number can only be given to the individual who “owns” that number. The medical record number cannot be given out over the phone. It can only be given in person by J.C. Blair registration or HIM staff after proper photo identification has been shown.
Can I access someone else’s patient portal?
At this time, the patient portal is only set up for individual patients, not for any other family member or medical power of attorneys, or proxies.
Will other healthcare facilities have access to my information?
No. The health records that are on your patient portal are records generated from J.C. Blair. You are not able to access records from another facility nor is another facility able to access records from J.C. Blair. To have your records sent to another healthcare provider, you may download, print and send or take your records to another facility or retrieve them from J.C. Blair’s Health Information Management (Medical Records) office by signing a release.
Will all of my records be included on my patient portal?
No. Any sensitive test results or behavioral health information will not be included on the patient portal but can be retrieved through the Health Information Management office (Medical Records).
What if I forget my password?
On the login screen on the patient portal, there is a “Forgot Password” link. Once you click on the link, a new temporary password will be generated. You may use the temporary password to access the portal. Once accessed, you will want to change the password to something you will remember for future login attempts.
I have a new email address. How do I change it on the portal?
By clicking on the Preferences button at the bottom of the Patient and Consumer Health Portal Homepage, users will be brought into a section where they can make these changes.
